Dermatophagoides farinae extract is a standardized allergenic preparation derived from the American house dust mite. It is utilized for the diagnosis and treatment of Type I hypersensitivity reactions, including allergic rhinitis, allergic conjunctivitis, and allergic asthma. As a diagnostic agent, it is used in skin prick testing and provocation challenges to identify sensitization. As a therapeutic agent, it is the basis for allergen-specific immunotherapy (AIT), administered either subcutaneously (SCIT) or sublingually (SLIT). The extract contains a complex mixture of proteins, most notably the major allergens Der f 1 and Der f 2. The mechanism of action involves immune modulation, shifting the immune response from a Th2-dominated allergic profile to a Th1/Treg-mediated tolerant state, characterized by the induction of allergen-specific IgG4 antibodies and the suppression of IgE-mediated mast cell and basophil activation.
