Dermatophagoides pteronyssinus allergen extract is a biological product derived from the house dust mite species of the same name, used primarily for the diagnosis and treatment of IgE-mediated allergic diseases such as allergic rhinitis, allergic conjunctivitis, and allergic asthma. As a diagnostic agent, it is employed in skin prick testing and provocation tests (nasal or bronchial) to identify hypersensitivity. As a therapeutic agent, it is used in allergen-specific immunotherapy (AIT), administered either subcutaneously (SCIT) or sublingually (SLIT). The mechanism of action involves modulating the immune system to induce long-term tolerance, typically characterized by a shift from a Th2-biased allergic response to a Th1/Treg-mediated tolerant response. This process includes the induction of regulatory T-cells (Tregs), increased production of the anti-inflammatory cytokine IL-10, and the generation of allergen-specific IgG4 antibodies that act as blocking antibodies to prevent IgE-mediated mast cell and basophil degranulation.
