Overview

Dermatophagoides pteronyssinus (Dpt) extract is an allergenic extract derived from the European house dust mite. It is a biological product used for allergen-specific immunotherapy (AIT) to treat allergic diseases in individuals sensitized to this common indoor allergen. [28, 30] AIT with Dpt extract is indicated for allergic rhinitis, allergic conjunctivitis, and allergic asthma. [12, 28] It has also been studied for its efficacy in atopic dermatitis. [30, 32] The therapy works by gradually administering increasing doses of the allergen to induce immune tolerance. This process shifts the body's immune response from a pro-allergic Th2-dominant state (characterized by IgE and inflammatory cytokines like IL-4 and IL-13) to a more tolerant Th1/Treg-dominant state. This modulation leads to the production of blocking antibodies (specifically IgG4), the induction of regulatory T-cells, and a reduction in the inflammatory response upon natural exposure to house dust mites. [12] The extract is administered either through subcutaneous injections (SCIT) or sublingually (SLIT) as tablets or solutions. [13, 18] It is also used as a diagnostic agent in skin prick tests to confirm sensitization. [26]
