The deruxtecan-based NaPi2b ADC is an investigational antibody-drug conjugate (ADC) designed to target Sodium-dependent phosphate transport protein 2B (NaPi2b, encoded by the SLC34A2 gene), which is frequently overexpressed in ovarian, endometrial, and non-small cell lung cancers. This specific ADC construct utilizes the deruxtecan (DXd) technology platform, which consists of a humanized anti-NaPi2b monoclonal antibody conjugated to a potent topoisomerase I inhibitor payload (exatecan derivative) via a tetrapeptide-based cleavable linker. The mechanism of action involves the selective binding of the antibody to NaPi2b on the tumor cell surface, followed by internalization and lysosomal cleavage of the linker to release the cytotoxic payload. The released DXd then induces DNA damage and apoptosis in the target cell and potentially exerts a bystander effect on neighboring tumor cells. In comparative preclinical studies, this deruxtecan-based formulation has been used as a benchmark to evaluate the efficacy and safety of next-generation NaPi2b-targeting agents.
