Deserpidine is an ester alkaloid drug isolated from the plant Rauwolfia canescens (family Apocynaceae). It has been used primarily as an antihypertensive agent for the treatment of high blood pressure and also possesses antipsychotic properties. Deserpidine acts by depleting noradrenaline stores in peripheral sympathetic nerve terminals and catecholamine and serotonin stores in the brain, heart, and other organs. This results in reduced blood pressure due to decreased cardiac output and peripheral resistance. Its mechanism of action involves inhibition of the ATP/Mg2+ pump responsible for sequestering neurotransmitters into storage vesicles within presynaptic neurons; neurotransmitters not stored are metabolized by monoamine oxidase (MAO), leading to a reduction in catecholamines[1][3][4][6]. Some sources also report that deserpidine competitively inhibits angiotensin converting enzyme (ACE), thereby reducing angiotensin II-induced aldosterone secretion[6][8].
