Desisobutyrylciclesonide-isothiocyanate conjugate, frequently referred to in research as compound 5c, is a novel multi-target hybrid small molecule developed for the treatment of asthma. It was designed by researchers at the Università degli Studi di Napoli Federico II by chemically combining desisobutyrylciclesonide (the active metabolite of the glucocorticoid ciclesonide) with an isothiocyanate moiety, which serves as a hydrogen sulfide (H2S) donor. This dual-action approach aims to integrate the potent anti-inflammatory properties of steroidal drugs with the cytoprotective, anti-inflammatory, and bronchodilatory effects of H2S. In preclinical mouse models of asthma, the compound demonstrated significant efficacy by inhibiting bronchial hyperreactivity, lung inflammation, and airway fibrosis, while also promoting the restoration of normal pulmonary architecture. The hybrid molecule is intended to address the limitations of chronic glucocorticoid use by providing synergistic therapeutic benefits in airway diseases.
