Overview

This is a combination of three small molecule drugs: - **Desloratadine** is a second-generation tricyclic antihistamine that acts as a selective antagonist of the histamine H1 receptor, used primarily for the relief of symptoms associated with allergic rhinitis and chronic idiopathic urticaria[2]. - **Cilostazol** is an antiplatelet agent and vasodilator, functioning as a phosphodiesterase III inhibitor (PDE3 inhibitor), indicated for symptomatic relief in intermittent claudication by inhibiting platelet aggregation and promoting vasodilation[1][9]. - **Dipyridamole** is also an antiplatelet agent and phosphodiesterase inhibitor, used to prevent postoperative thromboembolic events. It inhibits adenosine uptake/metabolism by erythrocytes and vascular endothelial cells, potentiating prostacyclin’s antiaggregating action[10]. A clinical study has investigated the use of this combination (desloratadine 20 mg/day + cilostazol 150 mg/day + dipyridamole 50 mg/day) in patients with chronic spontaneous urticaria (CSU) who have high D-dimer levels. The combination was found to be more effective than up-dosed antihistamines alone in reducing urticaria activity scores and D-dimer levels, suggesting benefit from combined antihistaminic and antiplatelet effects likely due to reduced platelet activation[3].
