Overview

Deslorelin is a synthetic decapeptide analogue of the naturally occurring gonadotropin-releasing hormone (GnRH). In a specific clinical strategy developed by City of Hope Medical Center, deslorelin is administered intranasally in combination with low-dose "add-back" estradiol and testosterone. This regimen is being evaluated in Phase II clinical trials for breast cancer prevention in premenopausal women at high risk, particularly those carrying BRCA1 or BRCA2 mutations. As a GnRH receptor agonist, deslorelin works by initially stimulating and then desensitizing the pituitary gland, which leads to a profound suppression of gonadotropin secretion and a subsequent reduction in ovarian estrogen production (ovarian ablation). The inclusion of add-back estradiol and testosterone is intended to mitigate the adverse effects of estrogen deprivation, such as bone density loss and menopausal symptoms, while maintaining the protective effect of ovarian suppression on breast tissue density and biomarker profiles.
