DeVIC is a combination chemotherapy regimen primarily used in the treatment of malignant lymphoma, especially for relapsed or refractory cases. The regimen includes four drugs: dexamethasone, etoposide, ifosfamide, and carboplatin. Each drug in the combination has a distinct mechanism of action: - **Dexamethasone** is a synthetic corticosteroid that suppresses inflammation and immune responses, acting through the glucocorticoid receptor to inhibit pro-inflammatory gene expression. - **Etoposide** is a topoisomerase II inhibitor, causing DNA strand breaks and preventing cell division. - **Ifosfamide** is an alkylating agent that crosslinks DNA, leading to apoptosis. - **Carboplatin** is a platinum-based agent that forms DNA adducts, resulting in cell death. DeVIC is most used in Japan, particularly for patients with relapsed or refractory lymphoma and for certain central nervous system lymphomas. It is administered in cycles and its safety and efficacy have been established in clinical practice and retrospective analyses. Common toxicities are those classically associated with the included agents (myelosuppression, nephrotoxicity, neurotoxicity, infections, and gastrointestinal symptoms).
