Devimistat + hydroxychloroquine is an investigational combination therapy studied primarily in relapsed or refractory clear cell sarcoma, soft tissue sarcoma, and advanced chemorefractory solid tumors. Devimistat (CPI-613) is a lipoate analog that selectively inhibits mitochondrial tricarboxylic acid (TCA) cycle enzymes, such as pyruvate dehydrogenase and α-ketoglutarate dehydrogenase, leading to disruption of cancer cell energy metabolism and induction of autophagy[1][5]. Hydroxychloroquine, an antimalarial and immunomodulatory agent, inhibits autophagy in cancer cells and may sensitize them to devimistat’s effects. The combination increases cellular stress, blocks tumor cell energy production, and potentially enhances the efficacy of chemotherapeutic agents while reducing their required dose[1][5]. Clinical studies evaluate the safety, maximally tolerated dose (MTD), overall response rate, progression-free survival, and overall survival of this regimen in multiple cancers[1][2][5][6][7].
