Dewallumab + Pexa-Vec is a combination immunotherapy regimen consisting of dewallumab (ZKAB001), a human monoclonal antibody targeting programmed death-ligand 1 (PD-L1), and Pexa-Vec (pexastimogene devacirepvec), an oncolytic vaccinia virus. Pexa-Vec is engineered to selectively replicate in cancer cells by deleting the thymidine kinase (TK) gene and to stimulate a systemic anti-tumor immune response through the expression of human granulocyte-macrophage colony-stimulating factor (GM-CSF). Dewallumab acts as a checkpoint inhibitor, preventing the interaction between PD-L1 on tumor cells and PD-1 on T cells, thereby restoring T-cell mediated anti-tumor activity. The combination aims to leverage the oncolytic virus's ability to turn "cold" tumors "hot" by inducing immunogenic cell death and local inflammation, which enhances the efficacy of the PD-L1 inhibitor. This combination is primarily being developed by Zhaoke (Guangzhou) Oncology Pharmaceutical for the treatment of advanced malignancies, including melanoma.
