Dexamethasone is a long-acting synthetic corticosteroid with negligible mineralocorticoid activity. It is fluorinated at position 9 and is approximately 25 times more potent than hydrocortisone. Dexamethasone acts by binding to the glucocorticoid receptor inside cells, leading to changes in gene expression that suppress pro-inflammatory mediators (such as eosinophils, mast cells, macrophages, interleukins) and upregulate anti-inflammatory mediators. This results in reduced synthesis of prostaglandins and leukotrienes and overall immunosuppression. Dexamethasone is used to treat a wide range of conditions including inflammatory disorders (such as arthritis and skin diseases), allergic reactions, certain cancers (like multiple myeloma), adrenal insufficiency, severe allergies or asthma attacks, ulcerative colitis flare-ups, nervous system disorders like multiple sclerosis exacerbations or myasthenia gravis crises.
