DHAX is a salvage chemotherapy regimen primarily utilized in the treatment of relapsed or refractory aggressive B-cell lymphomas, such as diffuse large B-cell lymphoma (DLBCL). It is a modification of the standard DHAP regimen, substituting cisplatin with oxaliplatin to mitigate specific toxicities like nephrotoxicity and ototoxicity while maintaining therapeutic efficacy. The regimen comprises three main components: dexamethasone, a glucocorticoid that exerts lympholytic effects; cytarabine (Ara-C), an antimetabolite that interferes with DNA synthesis; and oxaliplatin, a platinum-based alkylating agent that forms DNA adducts to inhibit replication. In clinical trials, DHAX is often evaluated in combination with novel immunotherapies, such as the CD20xCD3 bispecific antibody mosunetuzumab, to improve outcomes in patients who have failed prior lines of therapy.
