Dexamethasone is a synthetic glucocorticoid corticosteroid that suppresses normal pituitary corticotroph cells, while desmopressin is a synthetic vasopressin analog that stimulates ACTH release from autonomous (tumor) corticotroph cells. The combined administration (often called the Combined Dexamethasone Desmopressin Test or CDDT) increases specificity for detecting residual or recurrent pituitary adenoma activity by suppressing normal cells and selectively stimulating tumor cells.
