Overview

This is a **combination drug** consisting of dexamethasone, dexmedetomidine, and ropivacaine, commonly used as an admixture for regional anesthesia techniques such as nerve blocks. - **Ropivacaine** is a long-acting local anesthetic of the amide type, providing nerve block by inhibiting sodium influx in nerve fibers, thus blocking action potential propagation. - **Dexamethasone** is a glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressant properties, used here as an adjuvant to prolong the duration of nerve block and reduce postoperative pain. - **Dexmedetomidine** is a highly selective alpha-2 adrenergic receptor agonist with sedative, analgesic, and anti-sympathetic effects, used as an adjuvant to further enhance the duration and quality of regional anesthesia. When used together, dexamethasone and dexmedetomidine as adjuvants to ropivacaine have been shown to synergistically prolong the duration of analgesia, reduce postoperative opioid consumption, and decrease adverse effects such as nausea when compared to ropivacaine alone or with a single adjuvant[1][7]. This combination is most often administered perineurally (adjacent to nerves), especially for post-surgical pain control after procedures such as video-assisted thoracoscopic surgery (VATS), peripheral nerve blocks, and other regional anesthesia techniques.
