This combination therapy, developed and studied at the University of Faisalabad, is a multi-component "cocktail rinse" designed for the symptomatic management of oral lichen planus (OLP). The rinse contains dexamethasone (a corticosteroid), doxycycline (a tetracycline antibiotic), nystatin (an antifungal), and promethazine (an antihistamine). Dexamethasone provides potent anti-inflammatory action to resolve mucosal lesions. Doxycycline is included for its ability to inhibit matrix metalloproteinases, which contribute to tissue degradation in OLP. Nystatin serves as a prophylactic measure against oral candidiasis, a common side effect of topical steroid therapy. Promethazine is added for its sedative and local anesthetic properties to provide immediate pain relief. In clinical trials, this cocktail is typically evaluated as an adjunct to triamcinolone acetonide orabase.
