Overview

Dexamethasone is a synthetic glucocorticoid corticosteroid used as an adjunct treatment for acute migraine to prevent headache recurrence. In a specific clinical trial (NCT04112823) conducted by Montefiore Medical Center in collaboration with the Albert Einstein College of Medicine, intravenous dexamethasone (at doses of 4 mg or 16 mg) was evaluated in combination with 10 mg of metoclopramide, a dopamine D2 receptor antagonist. The study aimed to determine if a higher dose of dexamethasone provided superior sustained headache relief for patients presenting to the emergency department with moderate to severe migraine. Dexamethasone acts by binding to the glucocorticoid receptor, exerting potent anti-inflammatory effects that are thought to reduce the neurogenic inflammation associated with migraine, while metoclopramide provides antiemetic and abortive effects through dopamine receptor antagonism.
