Dexamethasone + radium-223 is a combination regimen used in investigational and off-label settings, primarily for the treatment of cancers with bone involvement. Dexamethasone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressant properties, commonly used to reduce inflammation and manage symptoms in cancer patients. Radium-223 (marketed as Xofigo) is an alpha particle-emitting radiopharmaceutical that mimics calcium and selectively targets areas of increased bone turnover, such as sites of bone metastases. Once localized to the bone, radium-223 emits high-energy alpha particles over a very short range, causing double-stranded DNA breaks in nearby tumor cells while sparing surrounding healthy tissue[1][3][6]. This combination has been studied particularly in metastatic castration-resistant prostate cancer (mCRPC) with symptomatic bone metastases[2][5][8], and also explored experimentally for multiple myeloma involving the bones[4]. Dexamethasone may be included to manage symptoms or as part of multi-agent regimens.
