Overview

**DOC** is a multi-agent chemotherapy reduction regimen comprising **dexamethasone, vincristine, and cyclophosphamide**. In the New York Medical College RADICAL pediatric lymphoma protocol, it is used as initial cytoreductive therapy before response assessment and subsequent risk-adapted treatment. Dexamethasone activates glucocorticoid receptor-mediated transcriptional programs and promotes lymphoid-cell apoptosis; vincristine disrupts mitotic spindle function by inhibiting microtubule dynamics; and cyclophosphamide is a DNA-alkylating prodrug that produces cytotoxic DNA crosslinks. The regimen is being evaluated within a Phase 2 targeted-immunotherapy strategy intended to reduce cumulative anthracycline exposure while preserving lymphoma control in children, adolescents, and young adults.
