Dexborneol is a bicyclic monoterpene compound and a stereoisomer of borneol, used as an active pharmaceutical ingredient with anti-inflammatory and neuroprotective properties. It is most commonly formulated in combination with edaravone (as in the drug Sanbexin) for the treatment of acute ischemic stroke. Dexborneol acts by inhibiting the expression of inflammatory cytokines such as TNF-α, IL-1β, COX-2, and iNOS in cerebral ischemia-reperfusion models. It also activates GABAa receptors, blocks oxidative stress pathways, reduces neuronal apoptosis and necrosis, improves cerebral blood flow, inhibits neuronal excitotoxicity and caspase-related apoptosis, and enhances blood-brain barrier permeability. These mechanisms contribute to its synergistic effect when combined with edaravone for neuroprotection after stroke[4][6].
