Dexlipotam tromethamine is the tromethamine salt of dexlipotam, the (R)-(+)-enantiomer of alpha-lipoic acid (thioctic acid). Developed by Jiangsu Aosaikang Pharmaceutical, it is primarily investigated for the treatment of diabetic peripheral neuropathy (DPN). Dexlipotam acts as a potent antioxidant and a vital cofactor for mitochondrial multienzyme complexes, including the pyruvate dehydrogenase complex and the alpha-ketoglutarate dehydrogenase complex. By enhancing mitochondrial energy metabolism and scavenging reactive oxygen species (ROS), it helps to alleviate oxidative stress and improve nerve conduction velocity in patients with diabetes. The use of the pure (R)-enantiomer is intended to provide better bioavailability and biological activity compared to the racemic mixture of alpha-lipoic acid commonly used in clinical practice. The drug is currently in Phase 3 clinical trials in China for both oral and injectable formulations.
