Dexmedetomidine + bupivacaine is a combination of two drugs frequently used in regional anesthesia and analgesia. Bupivacaine is an amide-type local anesthetic that blocks voltage-gated sodium channels on neuronal membranes, inhibiting nerve impulse transmission and providing local or regional anesthesia. Dexmedetomidine is a highly selective alpha-2 adrenergic receptor agonist that acts as an adjuvant to enhance the effects of local anesthetics by prolonging sensory and motor block duration, improving postoperative analgesia, reducing stress response, and providing sedation without significant respiratory depression. Dexmedetomidine’s vasoconstrictive properties may also delay systemic absorption of bupivacaine, further extending its effect[2].
