Overview

This is a combination of three intravenous anesthetic agents—dexmedetomidine, propofol, and remifentanil—used together for procedural sedation or general anesthesia. Dexmedetomidine is a selective alpha-2 adrenergic receptor agonist with sedative, anxiolytic, and analgesic properties that causes minimal respiratory depression. Propofol is a short-acting hypnotic agent that acts as a positive allosteric modulator of the GABA-A receptor to induce sedation and hypnosis. Remifentanil is an ultra-short-acting synthetic opioid analgesic that acts as an agonist at the mu-opioid receptor. When used in combination, dexmedetomidine can reduce the required doses of both propofol and remifentanil while improving hemodynamic stability during surgery and reducing postoperative agitation, anxiety, pain scores, and some side effects associated with higher doses of opioids or hypnotics[1][2]. This regimen has been studied in various surgical settings (e.g., urological procedures, endoscopic sinus surgery) to optimize anesthesia depth while minimizing adverse events such as respiratory depression or excessive bleeding[1][2].
