This is a combination of four agents commonly used in regional anesthesia and nerve blocks: - **Dexmedetomidine** is a highly selective α2-adrenergic receptor agonist with sedative, analgesic, and sympatholytic properties. It acts by inhibiting norepinephrine release both centrally (in the locus coeruleus) and peripherally, leading to sedation, analgesia, and reduced sympathetic tone. - **Ropivacaine** is a long-acting amide-type local anesthetic that blocks voltage-gated sodium channels on neuronal membranes, preventing nerve impulse conduction. - **Lidocaine** is an intermediate-duration amide-type local anesthetic that also inhibits voltage-gated sodium channels to block nerve conduction. - **Adrenaline (epinephrine)** is a non-selective adrenergic agonist added to local anesthetics for its vasoconstrictive effects; it reduces systemic absorption of the anesthetics at the injection site, prolonging their action and reducing toxicity. The combination of dexmedetomidine with ropivacaine-lidocaine-adrenaline has been shown in clinical studies to: - Prolong the duration of sensory and motor blockade - Improve postoperative analgesia - Shorten onset time for motor block without significant increases in adverse effects compared to standard regimens[2][6]. Dexmedetomidine enhances the effect primarily through peripheral α2-adrenoceptor-mediated mechanisms as well as central actions when absorbed systemically[6]. Adrenaline further prolongs anesthesia by vasoconstriction. This multi-agent mixture is used mainly for regional anesthesia such as brachial plexus blocks or other peripheral nerve blocks during surgery[2][6].
