Dextralink is a riboflavin-based ophthalmic solution used in corneal collagen crosslinking (CXL) to treat progressive keratoconus and corneal ectasia. Developed by the Ufa Eye Research Institute, the formulation typically consists of 0.1% riboflavin (Vitamin B2) and 20% dextran. During the CXL procedure, the solution is applied to the corneal stroma and subsequently activated by ultraviolet-A (UV-A) irradiation. This photochemical reaction generates reactive oxygen species that facilitate the formation of new covalent bonds between collagen fibrils, thereby increasing the biomechanical stability and stiffness of the cornea to halt disease progression. Dextralink is utilized in both standard epithelium-off protocols and transepithelial techniques, including delivery via iontophoresis.
