Dextrin-2 sulphate (Emmelle) is a sulphated polysaccharide polymer developed as a topical vaginal microbicide for the prevention of HIV transmission. It is a sulphated glucose polymer that acts as a polyanion, binding to the V3 loop of the HIV-1 gp120 envelope glycoprotein to block viral entry into host cells. Additionally, it has been proposed to induce the production of chemokines such as MIP-1α and MIP-1β, which block viral entry via the CCR5 co-receptor. Developed by ML Laboratories in collaboration with the Medical Research Council (MRC), it has been evaluated in Phase I and Phase II clinical trials for safety and efficacy in preventing HIV infection in sexually active women. It was selected over dextrin-6 sulphate due to its reduced anticoagulant activity while maintaining comparable anti-HIV efficacy.
