Overview

DF6002 + milvexian is an investigational combination regimen pairing DF6002, a long-acting interleukin-12 (IL-12) immunotherapy, with milvexian, an oral small-molecule factor XIa inhibitor anticoagulant, being explored in oncology settings where systemic immune activation coexists with heightened thrombotic risk. DF6002 is a novel IL-12-based cytokine therapeutic engineered by Dragonfly Therapeutics to enhance anti-tumor immunity, designed to stimulate cytotoxic lymphocytes and promote Th1-type immune responses against advanced solid tumors.[5][17] Milvexian, developed by Bristol Myers Squibb and Janssen, selectively inhibits activated factor XI (FXIa) to prevent pathologic thrombin generation while aiming to preserve hemostasis, and is in late-stage development for prevention and treatment of major thrombotic events including ischemic stroke, acute coronary syndrome, and atrial fibrillation.[1][3][8][10] The combination seeks to leverage DF6002-driven anti-tumor immune responses while mitigating cancer- and treatment-associated thromboembolic risk through milvexian-mediated FXIa blockade.
