This combination therapy involves the use of Double Filtration Plasmapheresis (DFPP) as an adjunct to the standard-of-care antiviral regimen of peginterferon alfa-2a and ribavirin. DFPP is an extracorporeal blood purification technique that separates plasma from blood cells and then filters the plasma to remove large molecular weight components, including hepatitis C virus (HCV) particles. The rationale is to rapidly reduce the baseline viral load before or during the initiation of antiviral therapy, potentially improving the sustained virologic response (SVR) in patients with genotype 1 chronic hepatitis C and high viral loads. Peginterferon alfa-2a acts as an immunomodulator and antiviral cytokine, while ribavirin is a nucleoside analog that interferes with viral RNA synthesis. This specific protocol was investigated by the National Taiwan University Hospital.
