DHBDC (dihydrobenzodioxepine cymserine) is a novel, small molecule experimental therapeutic being developed for the treatment of Alzheimer's disease. It functions as a highly potent and selective competitive inhibitor of human butyrylcholinesterase (BuChE). Structurally, DHBDC belongs to the dihydromethanobenzodioxepine class, which represents a novel series of agents that differ from traditional physostigmine-based cholinesterase inhibitors. The compound features a 4′-isopropyl-phenylcarbamate modification that provides its high selectivity for BuChE over acetylcholinesterase (AChE). By targeting BuChE, DHBDC aims to enhance cholinergic function in the brain, a key strategy for addressing cognitive decline in Alzheimer's patients. Preclinical studies have demonstrated that DHBDC possesses IC50 and Ki values that are likely attainable in humans, and it is currently positioned as a clinical drug candidate to evaluate its efficacy in human trials.
