DIA-615 is an orally bioavailable small molecule that acts as an agonist of the G protein-coupled receptor 119 (GPR119). Developed by the South Korean biotechnology company Diathegene, it was investigated for the treatment of type 2 diabetes and metabolic syndrome. GPR119 is primarily expressed in pancreatic beta cells and enteroendocrine cells (L-cells and K-cells) of the gastrointestinal tract. Activation of GPR119 by DIA-615 triggers a dual mechanism of action: it stimulates the glucose-dependent secretion of insulin from the pancreas and promotes the release of incretin hormones, such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), from the gut. This combined effect is intended to improve glycemic control, enhance insulin sensitivity, and potentially assist in weight management. While the GPR119 pathway showed significant promise in preclinical models, DIA-615 and similar agents have faced challenges in demonstrating sufficient clinical benefit in human subjects.
