Overview

Dialysate magnesium refers to the concentration of magnesium ions within the dialysis fluid used during hemodialysis. Magnesium is a critical electrolyte that serves as a cofactor for over 300 enzymatic reactions and plays a vital role in cardiac electrophysiology by stabilizing the resting membrane potential and modulating ion channel activity, particularly potassium and calcium channels. In patients with end-stage renal disease (ESRD) undergoing chronic hemodialysis, the concentration of magnesium in the dialysate is adjusted to manage serum magnesium levels. Research conducted by the University of Michigan has specifically investigated how varying dialysate magnesium concentrations (e.g., low vs. normal) affect the QT interval and QT dispersion, which are markers for arrhythmogenicity and sudden cardiac death risk in this population.
