Overview
Dian Dao San is a traditional Chinese medicine (TCM) topical powder formula composed of rhubarb (Da Huang) and sulfur. It originates from Wu Qian’s "The Golden Mirror of Medicine" during the Qing Dynasty. The formula is primarily used for treating acne vulgaris and other skin conditions characterized by heat, toxicity, and itching. Its main actions include clearing heat, cooling the blood, purging toxins, stopping itching, and killing parasites[1][2][4]. Modern research indicates that Dian Dao San exhibits antibacterial activity against Propionibacteribacterium acnes (now Cutibacterium acnes), a key pathogen in acne vulgaris[3][4]. Mechanistically, it appears to disrupt bacterial cell membranes and walls as well as interfere with bacterial glycolytic pathways by targeting enzymes such as gap (glyceraldehyde-3-phosphate dehydrogenase), pgk (phosphoglycerate kinase), and tpiA (triosephosphate isomerase)[4]. The formula has been recommended in Chinese clinical guidelines for external treatment of acne vulgaris.
