Dianhydrogalactitol is a first-in-class, small molecule bifunctional hexitol derivative with antineoplastic activity. It acts as a bi-functional N7-DNA alkylating agent that cross-links DNA via an epoxide group during all phases of the cell cycle, leading to disruption of DNA function and cell cycle arrest. Dianhydrogalactitol induces interstrand crosslinks at N7-guanosine residues in DNA, resulting in persistent double-strand breaks and subsequent cell-cycle arrest. It can cross the blood–brain barrier and has demonstrated clinical activity against various cancers including glioblastoma (GBM), central nervous system tumors, ovarian cancer, non-small cell lung cancer (NSCLC), bladder cancer, head and neck cancers, chronic myeloid leukemia (CML), and lung cancer. The drug is being developed by Kintara Therapeutics (now TuHURA Biosciences) among others[1][2][3][4][5][6][8].
