Diannexin is a recombinant homodimer of human annexin V (also known as annexin A5), developed as a protein-based therapeutic agent. It binds phosphatidylserine with high affinity and acts as a potent inhibitor of platelet-mediated events during thrombus formation. Diannexin exerts cytoprotective effects on endothelial cells by shielding exposed phosphatidylserines, thereby interrupting secondary microcirculatory inflammation and coagulation in ischemia-reperfusion injury (IRI). Its mechanism involves targeting ischemic tissues via phosphatidylserine binding, reducing inflammatory cell recruitment, oxidative stress, and tissue damage in various IRI models including liver, kidney, heart (myocardial infarction/reperfusion), and lung (acute respiratory distress syndrome). Diannexin has been investigated for use in kidney transplantation to prevent IRI but has not received regulatory approval[1][2][4][5][7][8].
