Dichlorphenamide, marketed under the brand name Keveyis, is an oral small molecule carbonic anhydrase inhibitor. It is indicated for the treatment of primary hyperkalemic periodic paralysis, primary hypokalemic periodic paralysis, and related variants. The drug functions by inhibiting the carbonic anhydrase enzyme, particularly types II and IV, which leads to a reduction in the formation of bicarbonate ions and protons. This action results in increased excretion of bicarbonate, sodium, and water, thereby influencing the body's acid-base balance and muscle cell excitability. This helps to stabilize muscle cell membranes and prevent the abnormal depolarization that causes episodes of muscle weakness characteristic of periodic paralysis. Strongbridge Biopharma initially acquired the US marketing rights for dichlorphenamide from Taro Pharmaceuticals North America, and later, Xeris Biopharma Holdings acquired Strongbridge Biopharma, expanding its commercial portfolio with Keveyis.
