Didanosine + stavudine is a combination of two nucleoside reverse transcriptase inhibitors (NRTIs), used as antiretroviral therapy for the treatment of human immunodeficiency virus (HIV) infection. Didanosine (ddI) and stavudine (d4T) both inhibit HIV reverse transcriptase by acting as chain terminators after incorporation into viral DNA, thereby preventing viral replication. This combination was historically used in both adults and children with HIV but is now largely discontinued due to significant toxicity concerns, including fatal lactic acidosis and pancreatitis, especially in pregnant women. The World Health Organization and other health authorities recommend against its use due to these risks[1][2][3][5][6]. The combination may still be referenced in older clinical studies or specific cases where alternative regimens are not available.
