DPC-817 (also known as PSI 5582 or D-D4FC) is a cytidine nucleoside analog developed as an antiretroviral agent for the treatment of HIV infection. It acts as a potent inhibitor of HIV type 1 replication in vitro and retains activity against viral strains resistant to lamivudine (3TC) and zidovudine (AZT), which are common components of highly active antiretroviral therapy (HAART). The drug is rapidly taken up by cells and converted to its active triphosphate form with an intracellular half-life of approximately 13–17 hours. Pharmacokinetic studies suggest that DPC-817 has low clearance and a plasma half-life longer than AZT or 3TC, supporting once-daily or twice-daily dosing. Importantly, it demonstrates minimal mitochondrial toxicity compared to other nucleoside reverse transcriptase inhibitors[1][2][4][5].
