Diepalrestat is an investigational **oral small-molecule aldose reductase inhibitor** being developed for **diabetic peripheral neuropathy**. Public trial records identify diepalrestat, also referred to by the development code **BNV-222** and in at least one registry presentation as **diepalrestat choline**, as a twice-daily oral therapy evaluated in a randomized, double-blind, placebo-controlled 12-month study in adults with diabetic peripheral neuropathy. Its intended mechanism is inhibition of **aldose reductase** in the polyol pathway to reduce hyperglycemia-driven sorbitol pathway flux and thereby slow deterioration in nerve function, including nerve conduction abnormalities associated with diabetic neuropathy. The available public evidence supports clinical development through a completed **Phase 2/3** study, with development associated with **NeuroMax**.
