Overview

Digoxin + amiodarone is a combination of two pharmaceutical drugs used primarily in the management of certain cardiac arrhythmias, especially atrial fibrillation (AF) and heart failure. Digoxin is a cardiac glycoside that increases myocardial contractility by inhibiting the sodium-potassium ATPase pump, leading to increased intracellular calcium in cardiac cells. It also slows conduction through the atrioventricular node, making it useful for rate control in AF. Amiodarone is a class III antiarrhythmic agent with multiple electrophysiological effects, including prolongation of action potential duration and refractory period across all cardiac tissues; it blocks potassium channels as well as sodium and calcium channels, and has noncompetitive beta-blocking activity. When used together, amiodarone can significantly increase serum concentrations of digoxin by inhibiting P-glycoprotein-mediated elimination pathways for digoxin. This interaction raises the risk for potentially life-threatening digoxin toxicity unless dosages are carefully adjusted and monitored[2][3][8]. Clinical studies have shown that this drug combination may be associated with increased all-cause mortality compared to use of either drug alone in patients with AF[1]. The use of this combination should be reserved for situations where benefits outweigh risks under close medical supervision.
