Dihydroergotoxine mesylate is a mixture of the methanesulfonate salts of three dihydrogenated ergot alkaloids: dihydroergocornine, dihydroergocristine, and alpha-/beta-dihydroergocryptine. It is an ergot derivative developed by Albert Hofmann for Sandoz (now part of Novartis). The drug has been used primarily to treat symptoms associated with age-related cognitive impairment, such as dementia and chronic pathological cognitive and neurosensorial impairment in the elderly (excluding Alzheimer’s disease), as well as ancillary treatment for conditions like Raynaud’s syndrome, peripheral arterial occlusive disease, visual disturbances of vascular origin, migraine prophylaxis, orthostatic hypotension, veno-lymphatic insufficiency, acute retinopathies of vascular origin, and sialorrhea in Parkinson's disease[5][6][8]. Its mechanism involves partial agonism/antagonism at adrenergic (alpha/beta), dopaminergic, and serotonergic receptors. Dihydroergotoxine modulates synaptic neurotransmitter levels and increases cerebral blood flow; it was once thought to act mainly via vasodilation but is now considered a metabolic enhancer[2][5]. The exact mechanism remains unclear.
