Diltiazem hydrochloride extended-release is a small molecule calcium channel blocker developed and manufactured by Ingenus Pharmaceuticals. As a benzothiazepine derivative, it functions by inhibiting the influx of calcium ions through L-type calcium channels in cardiac and vascular smooth muscle. This action results in peripheral vasodilation, reduced heart rate, and decreased myocardial contractility, thereby lowering blood pressure and reducing cardiac workload. The once-daily extended-release capsule formulation is bioequivalent to the reference drug Cardizem CD and is indicated for the management of hypertension, chronic stable angina, and angina due to coronary artery spasm. It is also utilized for rate control in certain cardiac arrhythmias such as atrial fibrillation and atrial flutter.
