**dimebon + donepezil** is a combination of two small molecule drugs investigated for the treatment of Alzheimer's disease. Dimebon (latrepirdine) is a former antihistamine repurposed for neurodegenerative diseases based on its potential neuroprotective effects, including stabilization of mitochondrial function and inhibition of neuronal cell death. Its precise mechanism is not fully established, but research suggests weak acetylcholinesterase inhibition, NMDA receptor antagonism, and additional mechanisms on mitochondrial function. Donepezil is an established acetylcholinesterase inhibitor approved for symptomatic treatment of Alzheimer's disease, acting mainly by increasing CNS acetylcholine via reversible inhibition of acetylcholinesterase. The combination aims to provide complementary or additive effects through these different mechanisms.
