Dimercaptosuccinic acid (DMSA), also known as succimer, is an orally active small molecule chelating agent. It is FDA-approved for the treatment of lead poisoning in children, where it functions by binding to heavy metals to form water-soluble complexes that are subsequently excreted in the urine. The Southwest College of Naturopathic Medicine (SCNM) investigated DMSA as an off-label therapy for children with autism spectrum disorder (ASD) who exhibit evidence of significant heavy metal exposure (including lead, mercury, and antimony). The rationale for this application is based on the hypothesis that heavy metal toxicity and impaired detoxification pathways contribute to the pathophysiology of ASD. A Phase 1/2 clinical trial (NCT00811083) evaluated the safety and efficacy of multiple rounds of DMSA administration in reducing autistic symptoms by addressing underlying metal burdens.
