Overview

Dimesna is the disulfide-linked dimer of mesna (sodium 2-mercaptoethanesulfonate). It is a metabolite formed when two molecules of mesna are oxidized and linked via a disulfide bond. Dimesna itself does not have direct cytoprotective activity but serves as an inactive circulating form that can be reduced back to active mesna in the body. Mesna and its metabolites—including dimesna—are used as uroprotective agents to detoxify harmful metabolites (such as acrolein) produced by chemotherapeutic agents like ifosfamide and cyclophosphamide. The mechanism involves chemical binding to these toxic compounds in the urinary tract, neutralizing their ability to damage bladder tissue[8]. Dimesna is rapidly interconverted with free mesna in plasma through redox reactions[8].
