Dirithromycin is a semisynthetic macrolide glycopeptide antibiotic used to treat various bacterial infections, including bronchitis, pneumonia (including Legionella and Mycoplasma pneumonia), tonsillitis/pharyngitis, otitis media, skin or soft tissue infections, and upper respiratory tract infections. It is a prodrug that is converted during intestinal absorption into the active compound erythromycylamine. The mechanism of action involves binding to the 50S subunit of the bacterial ribosome, inhibiting protein synthesis by preventing peptide translocation and stimulating dissociation of peptidyl-tRNA during elongation. This results in bacteriostatic activity against susceptible organisms such as Staphylococcus aureus (methicillin-susceptible), Streptococcus pneumoniae, Streptococcus pyogenes, Haemophilus influenzae, Moraxella catarrhalis, Legionella pneumophila, and Mycoplasma pneumoniae[1][2][3][4][5][6]. Dirithromycin offers once-daily dosing due to its long elimination half-life and achieves high tissue concentrations[8].
