Overview

Disitamab vedotin + durvalumab is a combination therapy comprising a HER2-targeted antibody-drug conjugate (ADC) and a PD-L1-directed immune checkpoint inhibitor. Disitamab vedotin (RC48) consists of a humanized anti-HER2 monoclonal antibody (disitamab) conjugated to the microtubule-disrupting agent monomethyl auristatin E (MMAE) via a protease-cleavable valine-citrulline linker. Durvalumab is a human IgG1 kappa monoclonal antibody that blocks the interaction of programmed cell death ligand 1 (PD-L1) with PD-1 and CD80. This combination is being investigated for its synergistic potential in treating HER2-expressing solid tumors, specifically biliary tract cancers like cholangiocarcinoma. The rationale for the combination lies in the potential of the ADC to induce immunogenic cell death and increase tumor antigen presentation, thereby sensitizing the tumor microenvironment to the effects of PD-L1 blockade and restoring T-cell mediated anti-tumor immune responses.
