Overview

Disopyramide is a class 1A antiarrhythmic agent used to treat life-threatening ventricular arrhythmias, such as sustained ventricular tachycardia. It works by inhibiting fast sodium channels in cardiac myocytes, thereby decreasing the rate of diastolic depolarization (phase 4), reducing the upstroke velocity (phase 0), and increasing the action potential duration in normal cardiac cells. This results in increased resistance of the heart to abnormal electrical activity and helps restore normal rhythm. Disopyramide also has anticholinergic and local anesthetic properties but does not block alpha- or beta-adrenergic receptors[1][3][5][6]. Its primary indication is for documented ventricular arrhythmias that are potentially life-threatening.
