Dithranol (also known as anthralin) is a synthetic small molecule used topically for the treatment of chronic plaque psoriasis. It works by slowing down the rapid proliferation of skin cells characteristic of psoriasis and has anti-inflammatory properties. The drug accumulates in mitochondria where it interferes with cellular energy supply—likely through oxidation that releases free radicals—impeding DNA replication and thus reducing excessive cell division in psoriatic plaques. Dithranol may also reduce elevated cGMP levels seen in psoriasis and induce lipid peroxidation while lowering endothelial adhesion molecules. It is available as creams or ointments at various strengths (typically 0.1–4%) and is applied directly to affected areas for short contact or overnight therapy. Dithranol can cause local irritation and stains skin/clothing but does not have systemic side effects typical of some other antipsoriatic agents[1][2][3][5][6].
