This combination therapy is being investigated in the Phase II EpiCardiAL trial (NCT02015312) for the treatment of cardiac dysfunction in patients with AL amyloidosis. The regimen combines standard-of-care cardiac medications—diuretics for fluid management and amiodarone for arrhythmia control—with epigallocatechin gallate (EGCG), a polyphenol found in green tea. EGCG is the primary experimental component, hypothesized to act as an amyloid fibril inhibitor by binding to and stabilizing amyloidogenic immunoglobulin light chains, thereby preventing their aggregation into toxic fibrils and potentially facilitating the clearance of existing amyloid deposits in the heart. The study is sponsored by the Fondazione IRCCS Policlinico San Matteo di Pavia and targets patients who have achieved a hematologic response to chemotherapy but continue to exhibit persistent cardiac impairment.
