The DKK1 dendritic cell vaccine is an autologous cellular immunotherapy designed to target Dickkopf-related protein 1 (DKK1), a secreted protein that acts as a natural antagonist of the Wnt/beta-catenin signaling pathway. DKK1 is frequently overexpressed in various hematological and solid malignancies, including multiple myeloma, where it contributes to tumor proliferation, immune suppression, and the development of osteolytic bone lesions. The vaccine is produced by isolating a patient's own monocytes, differentiating them into mature dendritic cells ex vivo, and then pulsing them with DKK1-derived peptides or transfecting them with DKK1 mRNA. Once administered back into the patient, these specialized antigen-presenting cells prime and activate the immune system to generate DKK1-specific cytotoxic T lymphocytes (CTLs). These T cells are programmed to recognize and destroy cancer cells that express the DKK1 antigen, potentially offering a high degree of specificity with low systemic toxicity.
